Product Simulation Cania has agreed to buy the Dental Insurance Plan D, with a monthly premium of IDR187,000. The benefits which Cania will receive are as follows:
100% coverage for visits to dentists and treatments that reduce dental pains if the dentist is a network member, or 70% if the dentist is not a network member;
100% treatment benefit for prophylaxis/scaling and cleaning/fluoride application for a 1-time visit in a year if the dentist is a network member, or 70% if the dentist is not a network member;
100% radiology benefit for a 1-time visit in a year if the dentist is a network member, or 70% if dentist is not a network member;
80% amalgam treatment benefit for up to 3 teeth in one year if the dentist is a network member, or 50% if the dentist is not a network member;
80% simple and complicated extraction benefit for 2 teeth in a year if the dentist is a network member, or 50% if dentist is not a network member;
80% root canal treatment benefit limited to 1-time procedure in one year if the dentist is a network member, or a 50% if dentist is not a network member;
IDR10,000,000 coverage limit in one year.
